Registration forms, the program handbook, and the medical release for the upcoming school year.

Forms and documents needed to enroll your student. New families complete the full registration packet; returning families submit the Student Intent Form to confirm participation.

How to register

Parent Night Orientation & Registration: August 28

Bring all signed forms with you to orientation.

  1. Fill out the online Registration Form.
  2. Download the Handbook, read it with your student, then print and sign the Handbook Agreement on the last page.
  3. Print and complete the Medical Release Form.
  4. Bring the signed Handbook Agreement and Medical Release Form to Parent Night Orientation.

Forms & Documents

  • 2025–2026 Registration Form (online form) — Required for all new students.
  • 2025–2026 Handbook (PDF) — Program policies, expectations, and the Handbook Agreement (last page) that families sign and return.
  • Medical Release Form (PDF) — Required for all students; print, sign, and bring to orientation.
  • 2025–2026 Student Intent Form — For returning students confirming participation. Link forthcoming — contact CCHB for the current form.
